Irrevocable Consent for Credit Checks and Data Disclosure
I/We hereby:
Consent and Authorization
- Provide irrevocable consent and authorization to you and any registered Credit Reporting Agency to conduct or continue conducting credit checks on me/us.
- Irrevocably consent to such disclosure and confirm that you shall not be held liable in respect thereof.
- Give unconditional and unequivocal consent to you and any registered Credit Reporting Agency to perform checks on me/us in accordance with the provisions of the Credit Reporting Agencies Act 2010.
- Provide irrevocable consent and authorization to you and any registered Credit Reporting Agency to conduct or continue conducting credit checks on me/us.
Authorization to Disclose Information
- Irrevocably authorize you to disclose and continue to disclose relevant information pertaining to, or arising from, or in connection with the facility or the security documents required hereunder, to your related companies as defined by Section 7 of the Companies Act 2016 or any of your associated companies (collectively referred to as the “LSD Prestamo Sendirian Berhad”), parties providing goods or services to any members of the LSD Prestamo Sendirian Berhad (“LSD Prestamo”), agents and their respective successors-in-title, to any other bureau or company established for the purposes of conducting credit checks, whether or not approved by Bank Negara Malaysia, governmental or regulatory body or authority, or to any other party or authority as you may deem fit or be requested to do so, at any time and without notice or liability.
- Irrevocably authorize you to disclose and continue to disclose relevant information pertaining to, or arising from, or in connection with the facility or the security documents required hereunder, to your related companies as defined by Section 7 of the Companies Act 2016 or any of your associated companies (collectively referred to as the “LSD Prestamo Sendirian Berhad”), parties providing goods or services to any members of the LSD Prestamo Sendirian Berhad (“LSD Prestamo”), agents and their respective successors-in-title, to any other bureau or company established for the purposes of conducting credit checks, whether or not approved by Bank Negara Malaysia, governmental or regulatory body or authority, or to any other party or authority as you may deem fit or be requested to do so, at any time and without notice or liability.
Indemnification
- Agree to indemnify you and hold you indemnified against all claims, actions, proceedings, damages, and expenses incurred by you or to which you may be subject as a consequence of your acting in accordance with the aforementioned authority and consent.

Extent of Consent
- This includes verifying and continuing to verify information provided by me/us or any other party(ies) to you, with any relevant party (including, but not limited to, any credit bureau, organization, or corporation established for the purpose of collecting and providing credit or other pertinent information) relating to me/us, at your absolute discretion and at any time.
- This consent extends beyond mere credit/loan processing; it encompasses review, evaluation, restructuring, obtaining new information, updates on me/us, legal documentation consequent to contract(s) or facility(ies), or any other proposed arrangement for as long as there exists a business relationship/credit facilities, and for any recovery of goods or litigation/contemplated litigation proceedings.
- This includes verifying and continuing to verify information provided by me/us or any other party(ies) to you, with any relevant party (including, but not limited to, any credit bureau, organization, or corporation established for the purpose of collecting and providing credit or other pertinent information) relating to me/us, at your absolute discretion and at any time.

- Acknowledge and agree to the contents of this notice.
